Note: If you apply within 30 days of your Benefits Eligibility Date, you will not have to take any medical tests or answer any health questions for certain levels of VTL and/or disability insurance, referred to as Guaranteed Issue amounts. If you apply after 30 days of your Benefits Eligibility Date or for amounts in excess of the Guaranteed Issue amounts, you will have to provide proof of good health. You can apply for AD&D insurance at any time without proof of good health.
Online Applications submitted on this website are made in Eastern Standard Time (EST). If applying for coverage during a guaranteed issue period, your application must be submitted by 11:59pm EST of the last day of your guaranteed issue period.
